RITM Catalog Item Change through flow designer

Currently, for certain catalog items where Flow Designer has been implemented, changing the item in an existing request successfully updates the item. However, the corresponding approvers are not added, and the details are not updated in the RITM.

Not sure i understand your question, why would you change the item on the RITM??

If a user accidentally creates a request and later needs to change the item, then it becomes necessary in such cases.

You shouldn't be changing the catalog item on the fly, the best approach is - Give requestor ability to cancel the request and ask them to reopen the request with the correct item.

If a user has mistakenly created a request with an incorrect item and wishes to update it, the item details should be modified accordingly.
