RITM goes straight to "approved" state

Ive been working on trying to solve this issue where after submitting a catalog item the RITM jumps straight to an "approved" state and instead it should be asking for approval. This is occurring even though an "ask for approval" action has been put in place within the flow.
Before the "Ask for approval" action is implemented I have imported all variables into the flow. One of those variables is manager (a field that is selected by the user to indicate their manager).

Hello,
Every request item goes through a request level approval process flow.
If that passes, then it's "approved" and then the RITM level process begins. Just stating that for the record as far as approvals go...however...in your case...
It's recommended to set up approve rules and reject rules, separately, just to ensure things operate properly, such as:
You need to ensure that the "manager" is properly defined...is this from a catalog item variable? Did you grab the catalog item variables before this step...so that those variable pills are available and you can dot-walk to the requested for's manager (for example)?

I'm glad your issue is resolved and it appears it was due to the manager definition as I mentioned above. If the manager did not contain the sys_id, the approval record wouldn't be utilized and thus this step would be skipped. Dot-walking to the manager from the requested for would help and you can use that pill.

Are you talking about the Stage of RITM going to Approved state or Approval itself getting set to Approved State?
