Service Catalog 'Order Now' is not working for some users

Hi, For some users when they click 'Order Now' button it redirects to the service catalog item form that they just submitted. It should be created a Request/Request Item numbers but it is not creating. What could be the reason?

Is this happing in Next UI, UI 16, or Service portal? If service portal did you create a custom SC Catalog Item Widget?

My instance version is Vancouver and it seems UI 16. But I am sure it is not in Service Portal. 

 

The issue is happening for some users and working for almost all the user.

Is it happening on all catalog items or only specific ones.

I have verified the issue is happening for other catalog items also. What could be the reason?
