Standard Change Template Fields not Showing up

Hello all,
Whenever we try to create a new standard template or modify an existing standard change template, we get the attached screen. The only field that appears is the template description and nothing else. Has anyone else had this issue or have any suggestions?

This is a record producer that is displaying a single variable. I would start by looking at the Record Producer for the New Standard Template (Service Catalog > Record Producers). Look for the Variables related list in the record.

Thank you for the reply. I have found that the New Standard Template producer does only have one variable. We will work on populating the variables we want on the producer.
When it comes to modifying or retiring Standard Templates, we do not get a dropdown or anything to choose what template to modify or retire, only the single Description variable. Do we need to create a variable or something that allows us to choose an existing template to modify/retire?
