Test cases format for incident process in Test Management Application

I want to specify test cases for incident process in Test Management Application. I'm a developer and new in writing the test cases. Would be appreciated providing any format of writing the test cases in Test Management Application.

First off I would create an Incident suite.  Put the incident-related test cases in your new suite.  Name the test cases so that they describe what it is that you're testing.
