How to add author name while creating major incidents in knowledge

While closing major incidents it's showing add knowledge if we clicked on that and later it's not showing author name in knowledge

This sounds custom, 

If you're talking about the "knowledge" checkbox, this is handled in the Business Rule "Incident Create Knowledge"

If you've got a button, you'll have to edit the button code to set it there.
