In the Classic UI, the Category Picker on a KB article has the + at the bottom to easily/quickly add a new category. The same functionality is not available in SOW. Before I raise a case with SNOW Support, could anyone please let me know if this is configurable or intentionally left out by SNOW by design?

No , the same functionality not available in SOW. Logging a case is good option.

They responded with "by design" and suggested adding an Idea for a future enhancement. However, I'm disenchanted with the whole Idea process since they moved it, and not everyone can get there.
Here's the Idea if anyone wants to vote it up, please do!
I found a way to navigate to the KB and the category list (including the New button) via SOW in a backhanded way so I'll probably do that. I avoid going to Classic UI as much as possible since we have adopted SOW for most of our daily work.

Hi @Vickie Runyon ,
I would highly discourage you in exposing that + to your Authors as that can lead to a nightmare taxonomy. I would suggest managing your categories at the KM/KCS Team level utilizing the Knowledge Base area to add them (native) but keep it simple. We do not allow our users to create random categories as we work with them to develop our overall taxonomy so the + is not visible. Should they require a new category, that is done via a request and a serious consideration by my team before adding. I hope that helps!
