Service Portal Custom widget

HI,

Any one Please suggest for this below requirement ,
Service portal :

Catalog item name : Computer Request 
Catalog form created with Mandatory attachment .
Now requirement is When User click the attachment tap its has to popup custom widget like "Do you want to Encrypt the Attachment or Not  " like this .
if user click yes then the attachment upload with encryption else attachment upload without encryption.

Kindly anyone suggest service portal widget code for this ....

are you using column level encryption?

If yes then your users should see this option while adding file? are they not seeing it?

They are not seeing it like need this popup how to add this popup with column level encryption????
