CEO Dashboard remove extra image

Just downloaded the latest CEO Dashboard release 3.0.1 from the store and there is an extra image displaying that is not on any of the store documentation. Unable to locate in UI builder.  Any ideas?  How can I reach out to Innovation Lab to report?
