SLA notificatiom

Hello, how do I make a notification under the following conditions:
Alert - SLA almost breached 75%
 sent to- a care provider
When was it sent-
SLA reached 75% of the continuation specified in the SLA definition

I would appreciate help on how to do the notification and show me what to fill in because I got confused, thanks!!!!!!

You can do this using workflow or flow designer.

Table Name : task_sla

You can check OOTB workflow "SLA notification and escalation workflow" for your reference.

I wanted a way to create a notification like I sent in the attached image

So when SLA hits 75% you need to send a custom notification ...??

Is my understanding correct...??

The notification is sent when the SLA has reached 75% of the duration specified in the SLA definition
