sla notificatuon flow

Hello ! I have an SLA flow ready, now I need to create an SLA that will check the following notification for me: I would like to know please how I create an SLA and how I fill in the fields and the data is - that the notification is sent to assignh to and also how I check that the notification is sent how do I do this?

Not sure i have understood the requirement here, could you explain whats the requirement here??

you would like to create a SLA?? or would like to configure notification for SLAs??
