Custom assessment questionnaire and approvals for a request exception

I have a requirement to add a custom assessment questionnaire and approvals for a request exception. While developing this, I was able to design the questionnaire and link it to the basic configuration. However, when I try to request an exception, I am not able to see the questionnaire pop up. Please let me know if anyone has faced the same issue.

Have you enabled "Enable questionnaire to request exception" from the Vulnerability Response > Administration > Exception Management menu?

I am trying to achieve this in the USEM workspace, but I do not see that option. However, I found the issue—it was because my assessment target was in a different scope. There is a flow action that creates the assessment, which was throwing an error. I changed the assessment target scope to match the assessment template, and it started working.
