Please upvote idea portal submission regarding multiple Exception Questionnaire options

Hi Everyone,

 

I submitted an Idea Portal idea that asks for an enhancement to the Exception Request Questionnaire form.

Currently, there is only one questionnaire you can have.  However, there are different types of 'Reasons' for an exception, each with different needs for a questionnaire.  Example: an exception request with a Reason of 'Risk Accepted' requires different questions about rationale and reasoning behind the request than an exception request with a Reason of 'Fix Unavailable' or 'Mitigating Control in Place'.  

In our scenario, we have about 15 questions we ask of the requester if the Reason is 'Risk Accepted'.  For Mitigating Control in Place, we have 1 question.  For 'Fix Unavailable' we ask 2 questions.

Currently, we only have the option to put all 15 questions in the request.  This frustrates requesters because most of them do not apply to them, unless it is the Risk Accepted reason.
