Process Automation Designer Activity

Hi everyone i have an issue using the process designer automation ... I am working with a Securtiy incident (SIR) by the Workspace using a playbook , i need a process automation activity that let's the user manual update the category field and sub category , how can i do it ? Please help me.

What i want to perform is shown in the picture i have uploaded in the attachments

@emidef94 Inside your user form activity for incident form, you need to add category and sub-category fields in the form fields
