Vulnerability Response Exception/False Positive Questionnaires Not Opening

After clicking Request Exception/False Positive and the completing the first popup, the VCA record and approvals are generated but the questionnaire does not appear. I have recently reverted the UI Page and UI Actions for an instance but there must be some lingering customization. The Exception Rules are set to require the questionnaires.
