Change Request values” have not been provided: Undefined

I had users reporting they were no able to request approval on Standard Changes. they would get 

Change Request values” have not been provided:  Undefined

What i ended up doing to fix this was removing all the fields from Mandatory Change Request values, then readd them. 

I no longer am getting that error. This problem is now solved, i wanted to post this incase it would help someone else.
