Issue with Catalog Tasks in a Standard Change

I have a Flow Design for a Request to Build a Server.

This flow has the action to create a Standard Change  using a Standard Change Template built for this,  and it populates the Request variables into the Change Form on customized fields.

This process on the Request side including creating the Change Record is working just fine.

The issue I have is that the Change Template has a Change Task that should be created when the record is created.

In Dev, it is doing that just fine.

However, in Test and Prod, which has the identical setup, is not creating that Change Task nor does the  Request Flow allow me to add it to create it and/or update it.

Any clue as to why this is broken?  It worked find until we upgraded to Zurich, but now both Dev and Test and are on Zurich so incompatibility is not the issue here.

Did you consider using a Flow instead of the legacy Workflow technology?

Yeah I am actually going to try that while I wait and see if there is a solution with the old workflow.  

I will update here if that "fixes" the issue
