Want a specific form public in Service Portal

Hi, I want a specific form public in the service portal. This form (catalog item/order guide) should be accessible to the public directly without any login to the portal/instance. It should also not affect any other forms which will remain private.

create a new page under portal 

add OOB widget related to catalogs to the newly created page and make sure tick the public checkbox on page.

add category on catalog item/OG related to portals to make them visible on portal form.
