What's an Easy Button for people to report incorrect data on the platform?

I would like ideas on how to implement an "easy button" that allows itil users to report data issues to the CMDB team. The easy button should be available in all the locations our key customers are likely to visit: configuration item forms, task forms (Incident, Change, Problem), dashboards we have created.
If the itil user finds "bad data" in any of these locations, they will click the easy button, type a brief note about the specific issue, and click Submit. (Simple for them!) The result should be a ticket to the CMDB team, telling us who clicked the button, where they were in the platform, and the exact issue being reported.
We already have something similar to this for CIs, but I don't know how to create something that can exist on task tickets and dashboards.
Have you considered or delivered this approach? Do you have any suggestions on an elegant way to accomplish this? I'm open to rethinking everything about the request. I just want something easy for our customers to report bad data.

I think creating a Global UI action (ex. Save is a Global Action) for this which would take user to a record producer to report bad data would be an ideal solution. I dont think this can be added to dashboards though.

I think you're right. But I'm holding out hope there's a way to get this functionality onto a dashboard.
