Empty Approvals are getting created

Hello,

In our servicenow instance, we are facing issue of getting empty approvals

there are 100+ empty apprpvals created from last month, and we cannot understand the source from its metadat.

all the fields are empty except created by(fields like approval for, approving, approver etc)

I tried to understand if this is created by flow or script but I am unable to trace anything.
I discarded possibility of these being created from workspace or like that, as if anyone is doing that then atleast 'approving/ approval from' gets populated

I am stuck in troubleshhoting this
any  help will be appreciated

please check server side scripts such as business rules , scheduled jobs or script includes ….etc

as I mentioned earlier, its hard to trace back itself
also this symotom is not being reproducible in lower domains and I cannot create a before business rule to get stack trace of the approval being created blank

 

I want to know is there any way to narrow down the investigation and have the exact direction of investigation
