How to add/configure a survey/assessment as a widget in Service Portal ?

I have a requirement to have assessment/survey visible on the Service Portal in widget , so that the user can submit the assessment directly through Service Portal.

Also please let me know how to configure the same on service portal.

Hi,

This is OOB and you dont have to do anything.

Just check if the property for survey on portal is activated or not.

You need to see this page on portal

https://INSTANCENAME.service-now.com/sp?id=my_surveys&sysparm_domain_restore=false&sysparm_stack=no

Also you need Service Portal - Survey Plugin activated.

i know it's a very old question, just in case if it helps others: "Surverys and assessments" is the widget used.
