How to debug IAccess Handler{} return=false issue on ACL?

Please help debugging below message for ACL. This message shows up when ACL Debugger is turned ON.

IAccess Handler{} return=false

Is this happening always when you debug the ACLs?

OR only for few tables etc

Not every time, it's happening only for specific table & specific user.
