RITM state not getting updated to close/cancelled on rejection of request. Also the REQ is in active state whereas RITM is inactive.
Please advice how do I correct this. Also this behavior is only for one catalog item whereas working fine for others.

You are using Approval action , why don't you use 'Set Value' activity like I have used ? I guess that's the reason why REQ is not closing. Also go through below KB article once

Even is I use set values the REQ till remain opens. Hence I will go with Stage as completed. The KB article is very helpful. Thank You very much for your inputs. Appreciate it!
