nobody can help you without any details nor visibility to your instance. Maybe you want to share some context: trigger of the survey, explain what you checked and what debugging was used etc.
It seems that the survey definition might be duplicated, but it's just a guess.

It is using OOB business rule and trigger condition is set to when state is resolved

Please modify the trigger condition state changes to resolved instead of state is resolved as it may causes the surveys to trigger multiple times.
