Survey is triggering multiple times when a ticket is resolved in a custom table

nobody can help you without any details nor visibility to your instance. Maybe you want to share some context: trigger of the survey, explain what you checked and what debugging was used etc.

 

It seems that the survey definition might be duplicated, but it's just a guess.

It is using OOB business rule and trigger condition is set to when state is resolved

did you check what's the trigger condition?

may be the record is getting updated multiple times leading to multiple surveys
