did you check what's the trigger condition?
may be the record is getting updated multiple times leading to multiple surveys

Hi @chitturijyo
There could be several possible causes :
1. KB0830680 Multiple Survey Instances Generated
2. Check Survey trigger source that it has not been called from multiple sources like flow etc
3. Check the Business rule's Trigger condition .
