I am trying to bring template automatically to description Field based on the CI on Incident Form.

Hi 

I have an requirement to bring the template into Description automatically based on the configuration item on Incident Form. So If I select CI as ServiceNow on the form ServiceNow related template should automatically come in the Description, till now we are manually selecting the templates so now we have do that automatically, Is there any way to do that.
