Hi Team,
We have requirement to create separate Risk Assessment section on change form not to use existing OOB Risk Assessment- which is done shared snapshot
list of question which needs to be placed under Risk Assessment section, based on question choices selected need to calculate Risk and populate existing Risk field as Low, medium, High
PFB snapshot, scores for each option in the questions updated
Once we select questions choices and perform any action (save, click on request approval) the risk should calculate based on the questions and choices present and risk should calculate according to the score shared in snapshot.

Change Management - Risk Assessment Summary
- Purpose: Captures information to calculate the risk of change requests.
- Activation: Requires role itil_admin and must be activated in Change Management.
- Assessment Process:
- Use a series of questions to evaluate risk.
- Employ a weighted score approach for each question.
- Calculate risk based on thresholds associated with the assessment.
- Configuration:
- Define multiple Risk Assessment definitions with one Metric Category.
- Ensure conditions are simple and mutually exclusive for clarity.
- Procedure:
- Navigate to Risk Assessments.
- Click New and fill out the form (Name, State, Description, Condition).
- Create Assessment Categories and Metrics.
- Set Assessment Thresholds for risk evaluation.
- Calculation:
- Risk score derived from weighted metrics compared against thresholds.
- Example: If the result is 7 and the moderate threshold is 6, risk is moderate.
