Unable to Open Submitted Records in Custom Form

I’ve created a custom table and form using ServiceNow Studio. The form submissions are working fine, and I can see the submitted records in the list view. However, I’m running into the following issues:

• When I click on any record to open it, I get the error:
  "The page you are looking for could not be found."

• When I try to preview the record using the (i) icon, it shows:
  "No preview available."

If I try to open the same records from within App Studio, they show an error (see attached screenshot).
