Flow Designer Error - Invalid values/ records

Some of our workflows run into error that we cannot locate the root cause and respective solution. We would like to know what those errors are and how we can fix them.
The below errors are using existing records which should not be invalid. And even if we submit the form on portal and trigger the flow, same error takes place.

Ensure your flow runs as System User.
if your variable is of type Reference to "Business Application Request" then perform a lookup first on that table with condition as
SysId = Get Catalog Variable->Variable->SysId
Then use "Update Record" action
