How to capture demand survey response on demand work notes?

I have a requirement where client wants to have,

1. Capture the Demand Survey responses and add them to the Demand - Work notes section
2. Capture the Risk score changes history on the Demand - work notes section

Hi Sandeep, thank you for response, above link doesn't give much information on my requirement,

could you please help me with a BR script like, 

Here, Demand survey response once completed it will store in task_assessment_detail table.

where task contains 'dmnd'> below attached details need to be copied on demand dmn_demand table work notes.
