ahoy @SS1510,
while this should be quite easy to do in a notification sent, I don't think there's a way to populate the Introduction note as it is for ALL the surveys sent to ALL the recipients based on various records in that Survey' scope..
But maybe I got it wrong, could you attach a screenshot to confirm where you want to add it?
Do you mean to have these details dynamically changed here?

@SS1510 I would say it's not possible.
If you want to add some context to what record is the survey related to, you can see it in the Portal (ESC) - more surveys to take, each has the corresponding request in my case:
If you select one of many, you see it here, there is this "Please help us improve..." message which is from the introduction section as seen in my previous post:
And users with backend access can see the REQ number in the top like this:
What you want to achieve is not possible in my opinion.

Hello @SS1510
I believe it is not possible and you should not try to achieve the same as well because you must send such Project related values in the e-mail so that end user can read them and then click the survey link which means end user is aware against which information/record he/she is providing the feedback.
The above won't require a flow as well because you can have trigger conditions set for your survey and ensure survey e-mail notification subject and body are appropriate to help end user understand.
