Catalog item for requesting system access, trying to add a workflow

I am creating a catalog item that is fairly simple for system access.  The user picks the system and has a text field to provide the access level/details.  Very similar to one we create for request a badge but the workflow is automatically completing the request for the system access and not doing that for the badge (which is the desired outcome).  Probably a simple error on my part but can't figure it out.  Any guidance is appreciated.

Please mark the below "Wait for Completion" as true in the create task, It will wait there in the task for the task to be complete.
