Service Catalog Variables not showing on the RITM

I have created a new service catalog item and have a Catalog UI Policy configured to display specific variables based on the selection on another variable. The variables are displayed correctly on the catalog item per the UI Policy and I am able to enter values, however when I look at the RITM, some of the variables are missing. I don't have any Scripts configured on the UI Policy. Attached screenshot shows the configuration. The only variable that displays on the RITM is the check_number. When I submit the same catalog item via the Service Portal all the variables display, see screenshot.

I'd welcome any insights as to why some of the variables are not displaying when submitted via the catalog. Thank you.

So this is a Simple fix.

On the Flow designer flow associated with the record add the "Get Catalog Items" from the catalog item and you should see the variables on the RITM under the summary.
