Forms task in Simulator - Welcome to ServiceNow (Course) Forms Task (Content)

I believe I should have completed all the steps correctly but still got validation failed - using both an existing instance and a newly-created instance. I've got no clues or suggestions on what might possibly went wrong. Any help would be appreciated.
