Help needed i am performing Lab 7.1.1 create UI Policy from the CSA but the policies are not working

Hi wondering if anyone can she some light. i have recreated lab 7.1.1 up until creating the UI policy and actions, but i cannot get them to work when creating an incident the two fields do not appear.

appreciate any suggestions , including a screen grap of the UI policy

Yeap i did just mentioned to Glidefather.. and attached incident grab.. ill try it one more time this morning and just continue as i want to finish this lab but would be amazing for any feedback or ideas as i was going crazy last night.

After a bit more digging. I went into form Design and added the two fields into the incident form and it worked!  from form builder i could not get it to work and on testing again it keeps placing the fields to the left of the form.
