I can't find the assigned to field on form builder. I selected All -> Organization -> manufacturers
Any suggestions?

You’re doing it incorrectly. You need to open the Incident form using Form Layout.
Create a new field and place it below the 'Assigned To' field.

Isn't the task to add the Contact field and not Assigned To?
