Post Xanadu upgrade portal issue

While submitting the custom form from the portal, we are facing some trouble.

Problem is form is not getting submitted and fields like office location and ISD code are getting empty and form is not submitting.

Button  "<button id="CreateBtn" class="btn btn-primary" ng-click="c.submitdata(); c.openModalS2();" >${Create New Generic Id}</button>" is not working.

This issue is happening after upgrade to Xanadu, while on other instance it is working fine(which is in washington).
