Auto creation of Assessment

Greetings Everyone;

In Servicenow SPM module; i want to create an assessment in a demand task with a state level condition
Scenario: When a new Demand task is manually or automatically created with states Pending, Open, Work in progress - a new assessment is to be created and assigned to the Assigned user.
Note - The assessment has to be auto created and attached to the Demand task record with its number auto populated in the task field

For now i have created an Assessment Metric Category, and Assessment Metric Type, but it need manual intervention to create assessments (buttons highlighted). I see that it is not manual and not getting attach to the demand task, how can i fix it?

i did set questions, and by clicking on the buttons the assessment s are getting created and assigned.

but it is not auto created, also i see that the task field in assessment instance is not being populated with the demand task number.

Are you creating them from the assessment? In that case, you aren't linking them. You should update the demand task and have that action create the assessment. It will then also be linked to the record.

However, be aware that this could possibly create 3 assessments, because you have 3 different states on which they should trigger...

We will be changing the condition to Closed Complete.

Can you help me where i can link them or what action is required for assessment creation.
