You may want to check a few things. First, verify that the Catalog Item is still active and accessible and that there were no recent changes to variables or permissions. Also review the Flow execution details in Flow Designer in ServiceNow to see if the Submit Catalog Item Request action is failing silently or returning an error.
If it previously worked, it’s also worth checking whether any updates, ACL changes, or workflow/flow modifications were applied recently that could prevent the RITM from being created.
