Password Reset not working in one environment while it works fine in other

Password reset in Password Reset Service Desk is not working. Whenever I try to reset a password it sows error. It is not working in one environment but works fine in another. I have compared the two to check why it is not working in one while allowing me to reset password in other. I have compared Business Rules and Modules of two but there is no difference in them.

Could you please check the logs:
Password reset -> Activity Logs 

Check what is causing the error so we can troubleshoot further.

I have checked for error in activity log and it inform that it is an out of bound exception.

I am getting exception out of Bound error. I have checked Business rules, Script Include, Ui pages and Flow. There is no issue in them and I couldn't identify where the error is and why I couldn't reset password.

did you check workflow/flow logs/execution list.
