Risk Assessment Form link is not working

In my PDI i have installed "Change Management --Risk Assessment"  and i have created a new risk assessment. Also created the assessment categories along with questions. All questions are here of yes/no and i have defined some value for yes and no, also based on them i have defined the threshold.
But when clicking on the Risk assessment on change form, no action is happening. and nothing is displayed.

I was just checking only the type in filter conditions.

Then i tried checking both type and model since our change request use models. and used the same conditions in assessment and assessment category.
Type is one of normal, Emergency AND
Model is normal OR
Model is Emergency
