We have a variety of users request IT Changes. The required process and artifacts are hard for those who do this task infrequently. Has anyone ever seen a form/UI like a question/inquiry dialog that would prompt the requester for needed fields and then populate the Change request? Including the required tabs like test validation and related CIs in the CMDB.
