I have attached the workflow below. Workflow is not working fine so it's misleading and also attached the context of mail which is sent to requestor as part of workflow.

@AlluP , check if there's any notification that is been sent out as part of the workflow or not.
If yes, remove the notification and adjust the workflow according to that.
