Flow Designer with Catalog Item

Here on the screenshot below a total of 4 options are there. When, I click on Application Access Required option , The catalog task should be created with short description 'Test 1'. 

like wise, 

when I Click on Application Access Removal Required option, The catalog task should be created with short description 'Test 2'. 

same for Test 3 and Test 4. 

How to do this in flow designer.
