Can we create multiple CI's records by submitting one catalog item form?

I need to create multiple CI's records for Two CMDB CI class table (cmdb_ci_db_mssql_instance & cmdb_ci_cloud_database) . I have automated this catalog item by flow , in flow I have mapped catalog item variables with table fields.  Now, it is creating only one record as we are using "Name" variable type as single line text. But the requirement is to create multiple CI's by submitting one request only. Is it possible to create multiple CI's by one request? Please find the attached screenshot of catalog item form and flow designer.
