Short answer: there's no single out-of-the-box "CMDB maturity questionnaire" table you can just switch on — but you don't need one, because the assessment already exists in two halves, and the value is in combining them.

The quantitative half is the CMDB Health dashboard. Completeness, Compliance, and Correctness are your objective maturity baseline — don't self-assess data quality when the platform already measures it for you.

The qualitative half is a maturity model mapped to CSDM's Crawl → Walk → Run → Fly stages. Score each CSDM domain (Foundation, service modeling, portfolios) against what your organization actually does today versus what the stage requires. You can operationalize this natively with the Assessments module (Assessment Designer / metric definitions), so the questionnaire lives in the platform and is repeatable — not a one-off spreadsheet.

One caution from running a lot of these: the questionnaire is the easy part. Most CMDB maturity efforts stall not because the checklist is missing, but because CSDM gets treated as a data-model migration instead of an operating-model change — a governance and skills gap, not a technical one. Anchor every question to an owner and a process, not just a table.
