Discovery Finds IPs but CI Fields Are Empty and No Relationships Detected

Hi ServiceNow experts,
I'm having an issue with Discovery. It finds both active and inactive IP addresses, but the CI fields are showing up empty. It's not discovering all the CIs, and for the CIs it does find, there are no relationships or dependency views.
Some IPs are active, but Discovery still doesn't catch their CIs. All of my devices have unique names, so I thought at least the names would be discovered, but they aren’t.
Any idea why this is happening? How can I get Discovery to fill in the CI fields and also find the relationships between CIs automatically?
