Field is not visible neither on a form nor in XML

We have a problem with one field ip_address. 

Field is added to the form in a default view through form layout

This field is also added via form designer

We've checked client scripts, ui policies, acls, dictionary entry, table settings as well - nothing, that could possibly cause this. 

Field is not visible in XML and is missing on a form.

But I really think you have to create a case for this. I doubt this is something we can help with.

I am very interested in the outcome of the case though.
