Hotfix & patches are small or some time medium sizes fix of issue which occourced in previous release (s). In patches you may get new funcationality as well like on D.c. Release Patch 3 Hot Fix 1 - SN release creator studio in PDI.

Although you didn't ask me: if you read the email that is send when a patch or hotfix is released, it already tells you the answer. It is to fix bugs and keep the instance(s) secure. As with every piece of software, bugs are part of the system and ServiceNow is not any different.
